The name multiple sclerosis refers to the scars (sclerae – better known as plaques or lesions) that form in the nervous system. These lesions most commonly affect the white matter in the optic nerve, brain stem, basal ganglia, and spinal cord, or white matter tracts close to the lateral ventricles. Peripheral nerves are rarely involved.
The exact cause is unknown. However, a number of genetic variations have been shown to increase the risk.

In a developed case, Spastic Paraplegia (impairment of the motor and sensory function of the lower extremities with spasticity of the muscles) is the commonest picture. But signs of spastic paraplegia with one of the symptoms like nystagmus (involuntary eye movement), intention tremor (a trembling of a part of the body when attempting a precise movement) or scanning speech (abnormal speech in which the person pauses between words, breaking the rhythm of the phrase or sentence) are suggestive of MS.
MRI and Myelography are useful in the diagnosis.
